DocuMed, Inc. is offering our Continuity of Care Document (CCD) reader to clinical practices that currently do not have an EMR system.  DocuMed’s CCD Reader can open a CCD for review and printing, and it allows users to store the document electronically in a human readable format.

 

What is a Continuity of Care Document (CCD)?

  • The Continuity of Care Document (CCD) is the result of a collaborative effort between the Health Level Seven (HL7) and ASTM organizations to “harmonize” the data format between ASTM’s Continuity of Care Record (CCR) and HL7’s Clinical Document Architecture (CDA) specifications. 

 

What does this mean to you?

  • The CCD will enable referring physicians, urgent care, and emergency departments to send the patient summary containing a core data set of the most relevant administrative, demographic, and clinical information facts about a patient's healthcare, covering one or more healthcare encounters.  It provides a means for one healthcare practitioner, system, or setting to aggregate all of the pertinent data about a patient and forward it to another practitioner to support the continuity of patient care. Its primary use case is to provide a snapshot in time containing the pertinent clinical, demographic, and administrative data for a specific patient.

 

More and more primary care physicians are embracing EMR technology that has the capability of producing a CCD, however, because most referral physicians do not currently have the capability of opening, printing or storing of the CCD, they are not sending it.  Once you have downloaded DocuMed’s CCD reader, you may notify your referring practices that you would prefer receiving patient information via electronic CCD
